What is an immigration physical?

An immigration physical is a required medical exam for people applying for a green card in the United States. A green card allows people with an immigrant visa to gain permanent residency in the United States. 

During an immigration physical, a civil surgeon — a medical doctor with special certification from US Citizenship and Immigration Services (USCIS) — performs an exam to verify that you meet certain medical requirements needed for permanent residency.

What do I need to bring to an immigration physical?

Washington Internal Medicine provides a list of necessary documents ahead of your immigration physical. These include:

Form I-693 (Report of Medical Examination and Vaccination Record)

You should fill in parts 1-4 of Form I-693, but don’t sign it. Make sure your name is on every page. 

Stamped envelope

Bring a stamped envelope with your current address as the return address, and the mailing address as the USCIS address provided on Form I-693. 

Identification

Bring government-issued photo identification such as a passport, driver’s license, or other official photo ID to your immigration physical.

Vaccination record

Bring all vaccination records to your immigration physical. This includes vaccinations like DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus, and meningococcal disease.

Exam fee

Washington Internal Medicine provides an exam fee schedule ahead of your exam. You pay a basic exam fee for the examination and medical documentation. Any additional vaccinations you may need are an additional cost.

What does an immigration physical exam include?

The exam includes a medical history review, comprehensive physical, chest X-ray, blood test, and urine test. You may need vaccinations if you’re not up to date. 

After your physical exam, you receive a copy of the results along with the sealed envelope containing Form I-693. Don’t open the envelope, as this invalidates the results.
